THE OPPORTUNITYAs the Bed Allocations Clerk, you will report directly to the Patient Flow Coordinator. Your responsibilities will include the placement and organization of current hospital admissions, including interfacility transfers and repatriations. Additionally, you will be involved with registering and transferring patients admitted to the hospital through the Emergency Department into the Meditech system. Tracking open and closed beds in the hospital and reporting this information to the Coordinator for bed meetings. You will be maintaining up-to-date patient records by processing order and entry requests from nursing units, including transfers between floors within different programs, is also part of the responsibilities. You will be involved with ensuring that ALC (alternate level of care) patients are admitted in Meditech Expanse and their records are up to date. Additionally, serving as the main registration contact for the GBGH @ Home program. KEY ACCOUNTABILITIES & RESPONSIBILITIES• Process admissions from the Emergency Department, as well as direct admissions from the community, repatriations, etc., as directed by Patient Flow, Team leaders or Hospital Coordinators. • Participate in daily bed round meetings with, clinical managers and team leaders.• Assign beds and communicate with floors and other organizations about bed placement requirements• Coordinate with IPAC to determine patient appropriateness of placement. • Assign Hospitalist to patients as appropriate, keeping the hospitalist workload balanced. • Organize and distribute admission paperwork to correct areas.• Repatriation lead: manage incoming repatriations from other facilities, answer calls, faxes, and emails regarding incoming and outgoing transfers to the hospital.• Keep Critical up to date daily and report to Coordinator any totals for bed meeting and/or director escalation. • MOH bed censes daily, updating online. • ALC patient registration, once receives approval from Coordinator/Director – maintain ALC census and ensure accuracy. Report to Coordinator if number census is not accurate, or over allowance noted.• Manage GBGH @ Home email for registration and discharges. Register/Discharge patients in Meditech. • Keep an up to date GBGH @ Home census, working with the Coordinator and PFNs. • Keep GBGH @ Home tracker up to date for government reporting responsibilities.• Answer telephone inquiries and monitor the GBGH @ Home email for incoming referrals. • Maintain daily census in Meditech bed board, using signed nursing unit census and informing Coordinator of any discrepancy.• Coordinate with other hospital departments (i.e. Environmental Services and Patient Transport) to ensure that patients are moved safely and efficiently.• Communicate with Team leaders to ensure bed needs are met• Ensure that appropriate documentation has been completed accurately for expired patients (Death Certificate).• Maintain Death Log and manage morgue key, according to department policy.• Knowledge and ability to implement computer downtime procedures, ensuring manual inpatient lists are continually updated. • Assist with improvement initiatives as required.

Job Requirements:

QUALIFICATIONS & EXPERIENCEThe ideal candidate will possess the following key competencies, record of experience and academic preparation:• College Diploma in Medical Administration or Equivalent• Active member of Ontario Hospital Admitting Registration Association• Two years experience in a similar capacity• HIM Professional via CHIMA (Canadian Health Information Management Association) preferred• Excellent communication skills to relate to a variety of different personalities• Demonstrates excellent customer service skills• Demonstrates good problem-solving skills• Able to organize own time and perform duties under minimal supervision• Facilitation skills and work effectively as a team member• A comprehensive understanding and demonstrated computer skills• Demonstrated ability to work independently, be self-directed and manage competing priorities.• Demonstrated strong communication and interpersonal skills, with the ability to work collaboratively with physicians, inter-professional team members, and other stakeholders • Embraces a philosophy of person-centered care while being committed to GBGH’s Values and Purpose Statement • Ability to adapt to change in a fast-paced working environment • French verbal and written communication skills are considered an asset in this position HEALTH, SAFETY & WELLNESSJoin us in a culture where your well-being is a top priority.
